Frequently Asked Questions

 

I ONLY HAVE ONE (1) / I DON’T HAVE ANY OF THE MENTIONED VALID IDENTIFICATION CARDS?
You may use a valid POSTAL ID, COMPANY ID, or SCHOOL ID as an alternative; however, it must be supported by at least two (2) or three (3) of the following documents:

  1. Voter’s Affidavit from Comelec – Intramuros
  2. Form 137 with school dry seal
  3. Year book
  4. Social Security System (SSS) E1 form
  5. Old cedula / old National Bureau of Investigation (NBI) clearance
  6. Insurance policy
  7. When applicable, you must present the National Statistics Office (NSO) birth certificates from two (02) of any of your children and /or siblings

 

WHAT IF I HAVE NO RECORD FROM NATIONAL STATISTICS OFFICE (NSO) / THE RECORD WAS DESTROYED?

You must secure a Certificate of Destroyed or No Record from NSO and your Local Civil Registrar (LCR). For applicants born in 1949 or earlier, you must also secure a Joint Birth Affidavit of two disinterested parties with copy of their IDs.

 

I’M RENEWING MY OLD (GREEN) FILIPINO PASSPORT (PRE-MACHINE READABLE PASSPORT) WHAT ARE THE REQUIREMENTS?

Requirements will be the same as renewing a passport although you will have to provide original copy of your Birth Certificate from the Nationals Statistics Office (NSO).

 

MY PASSPORT GOT MUTILATED / DESTROYED / LOST / STOLEN. WHAT ARE THE REQUIREMENTS TO SECURE A NEW PHILIPPINE E-PASSPORT?

For Mutilated / Destroyed Passports, you need to present an Affidavit of Mutilation and all requirements stated in New Passport Applicants.

For Lost / Stolen Passports, you need to present an Affidavit of Lost and all requirements stated in New Passport Applicants. If lost passport is still valid, a police report is required as well.
